Summary
This clinical trial is looking at a new way to treat B-cell acute lymphoblastic leukaemia (B-ALL) in children, teenagers, and young adults (under 25 years).
It focuses on people whose leukaemia has specific gene changes called Philadelphia chromosome positive (Ph+) or Ph-like (ABL-class).
The study is testing whether combining:
- Blinatumomab (an immunotherapy)
- with targeted drugs (dasatinib or imatinib)
- along with standard chemotherapy
works better than current treatment.
Researchers want to see how long patients stay well without the cancer coming back (event-free survival), overall survival, and how safe the treatment is and what side effects occur.
Who can take part?
- Children, adolescents, and young adults under 25
- People who have newly diagnosed Ph+ or Ph-like B-ALL
- People who:
- have not received treatment, or
- have only started early induction treatment
What does taking part involve?
Treatment is given in several phases over about 2 years, including:
- Induction (Parts 1 and 2):
- standard starting chemotherapy, followed by additional chemotherapy with targeted therapy (dasatinib or imatinib).
- Blinatumomab treatment (Blocks 1 and 2):
- blinatumomab immunotherapy is given intravenously (via IV) over about 4 weeks, along with targeted therapy and some additional chemotherapy.
- Interim maintenance 1:
- lower-intensity chemotherapy with continued targeted therapy and scheduled high-dose methotrexate.
- Blinatumomab treatment (Block 3):
- a further cycle of blinatumomab is given.
- Delayed intensification (Parts 1 and 2):
- chemotherapy becomes more intensive again.
- Interim maintenance 2:
- another period of lower-intensity treatment with chemotherapy and targeted therapy.
- Maintenance therapy:
- longer-term, lower-dose treatment chemotherapy, continued targeted therapy and occasional IV or spinal treatments.
Patients will also take some medicines by mouth and some through a drip or injection, have regular hospital visits, and have tests such as blood tests, bone marrow samples, lumbar punctures, and heart scans.
