Summary
This study is testing whether adding the drug cabozantinib to standard chemotherapy can improve outcomes for people with newly diagnosed with osteosarcoma.
Cabozantinib is a targeted medicine that works by blocking signals that cancer cells use to grow and to form new blood vessels, which may help slow or stop tumour growth.
Standard chemotherapy in this trial includes methotrexate, doxorubicin, and cisplatin (known together as MAP). These drugs work by killing cancer cells or slowing their growth.
Researchers want to find out if combining cabozantinib with MAP works better than MAP alone.
The study has two parts.
Feasibility Phase (now closed to recruitment as of 05/09/25)
This part tests different doses of cabozantinib to make sure it's safe to use with standard chemotherapy.
Efficacy Phase
This phase is comparing chemotherapy with or without cabozantinib to see which works better.
Participants are grouped by risk level (standard risk or high risk) and within that group are then randomly assigned (by chance) to a treatment arm.
Standard risk osteosarcoma
- Arm A (Standard treatment): MAP chemotherapy, surgery, then further chemotherapy.
- Arm B (Standard treatment + cabozantinib): MAP plus cabozantinib before surgery, followed by chemotherapy with cabozantinib and then cabozantinib alone as maintenance.
High-risk osteosarcoma
Treatment is the same as for standard risk:
- Arm C: MAP chemotherapy, surgery, then further chemotherapy.
- Arm D: MAP plus cabozantinib, surgery, and cabozantinib maintenance treatment
